Our office will verify your insurance benefits for you if you schedule your visit at least two days in advance. If your insurance company is not listed above, you may still have out of network benefits. Call for details.

Currently, United Healthcare and Aetna are not accepting any new chiropractic doctors as preferred providers on their plans. If you would like Dr Bomben to be a preferred provider for one of these companies, contact his office for more information.

If Dr Bomben is not a provider for your insurance plan, he is committed to competitive pricing as an out-of-network provider.

Cash Plans & Financial Hardship

Cash plans are available. These plans provide significant savings (up to 30%) for those who do not require our staff to file insurance claims. A typical office visit for a self-pay patient averages $40-120 per treatment. Additional financial hardship, senior citizen, child, and wellness plans are available to minimize your out-of-pocket expenses. Dr Bomben has never turned away a patient for financial reasons.

Payment Methods

Performance Health accepts cash, checks, and all major credit cards. Payment plans can also be arranged in advance.
